Chief - In theory, with a 5th electrode, there's no reason to use a dipole to mimic a precordial lead. With a 3-lead you can only view one lead at a time because the left over lead defaults to the ground lead. That's why you could only get one ch...

This is interesting. I just received correspondence this week from a medical team in Antarctica asking how to "modify a LP10 to obtain a 12-lead ECG". I explained to them how to obtain MCL-1 through MCL-6 and place the LP10 into "diagnostic" mode ...

I'm a fan of Garcia and Holtz, 12 Lead ECG - The Art of Interpretation. I also have a copy of Chou for reference. Good luck! Tom

Sorry, Josh. I thought that was pretty funny, but I don't want it to show up anytime someone Googles "interview with Wake County EMS". Thanks, Tom

Dr. Ray Fowler uses DKA in one of his capnography lectures and is pretty adamant that a low CO2 does not rule out DKA. http://www.doctorfowler.com/www/lectures/capnographyforEMSToday2008.ppt (BIG FILE) Slide 102-104. Tom

About Me
My name is Tom Bouthillet.

I'm a Lieutenant / Paramedic with Hilton Head Island Fire & Rescue.

I've been in the fire service since 1997 and I've been a paramedic since 1995.

I'm an ACLS instructor and I've taught Acute Coronary Syndromes and 12 Lead ECG interpretation around the country in various UMBC-affiliated Critical Care Transport (CCEMT-P) courses and as a consultant.

I'm co-moderator of the EMS-to-Balloon (E2B) Challenge listserv at Yahoo! and the EKG Club listserv at Yahoo!

I started the Prehospital 12 Lead ECG blog in October 2008.

In January 2009 I was invited to be a member of the EMS Advisory Committee for the SC Chapter of the AHA's Mission: Lifeline (STEMI initiative).

Feel free to leave me a message.

Tom,

Sorry for the belated reply. I didn't get it until today. I check my e-mail regularly but as of late haven't been checking my JEMS connect. As some have complained, some of the boards have been degenerating into Jerry Springerish type of slamfests, so I've been taking a break. Thank you for your comments. My hope is to make EMS a true profession, one in which the career-oriented can flourish without having to think about where they "really want to be", such as a police department, fire department, nursing school, etc.

Thanks again!!

You seem like an awesome, knowledgable instructor. I'd be honored if I could add you on here. I've been a Paramedic for about seven months and have had to go way over my head to try to become better than the average medic in these parts. I'm hoping to get my critical care certification when I hit my first year. There are some 12 leads I would love to pick your brain about!

Hey Tom,

We have relatives in New Jersey and Philadelphia. That's about as far south as we go. You're right, the name isn't very common here. I'm told that it's pretty common in Germany, like Smith is here. Be well.

I got bored watching the Super Bowl...Pittsburgh wins again, yadda, yadda.

I had a bunch of invites for this one, but didn't join until I saw a couple of threads that interested me enough to respond.

How was the USAR class?

Hi Tom, my name is Carl Mare' I'm an EMS Captain with Tucson Fire. I am the on-shift guy responsible for the ERLink project. The chiefs do the administrative stuff, I'm on the ground working with the medics and the equipment.
ERLink enables us to stream real-time audio and video from our paramedic units to the trauma center. We have cameras mounted inside the module and have plans to mount external cameras should funding hold up. We have 18 medic trucks in our fleet, all but 3 have the equipment installed and functioning. We should have all our trucks equipped in the next few weeks.
This is a joint project involving TFD, University Medical Center and the City of Tucson Department of Transportation. Funding is through a federal grant procurred by the transportation department, they were the primary department involved in setting up the wireless mesh network.
Anyway, we are the first to use telemedecine to this extent so we are the first to experience all the "bumps in the road". We now have the equipment working as designed and the network is getting stronger every day. Things are looking good.
I'd be happy to answer any questions you may have.

Yes, ER-Link provides a platform for video and audio feed from the PM Ambulance to the trauma center. CarePoint is the receiving point, Rosetta is the transfer point and the Mesh Network is the network for connectivity. Now, that's from me, not the guys who know the system well! Our best contact would be Capt. Carl Mare', I'll forward him your contact info. Thanks for the interest, Norma

tom,
I read your "What is a good Medic" from Dec. 14 and made a copy to share with my crews. I'm an RN employed by a large metropolitan fire-based EMS agency, and responsible for over 700 EMS providers including over 260 medics. With your permission, I'd like to include this as part of my "Paramedic Promotional Packet"...I really think this sets the stage for professional growth, responsibility and quality patient care. Thanks for sharing.

As a newly-licenced paramedic, I also agree that the current system is flawed. My clinicals alternated between extremely good and a few situations where I was the most-senior person on the truck...as a student. The problem isn't limited to the civ...

doc, You are also forgetting the relative call volumes and the effect they have. Law enforcement is the busiest function (crime fighting) so the cops have the least time to devote to other, non-crime-fighting functions. That makes it intutively o...
