When I am asked if I am a ambulance driver, I come back with "they won't let me drive I have to ride in the back with the patient" Our service employs 1 emt-p and 1 emt-b per unit. Talk about being treated at least equal or close to it we have the...

I agree with Chance on this one..... As to your main question, the only thing I would say different is that I would be more worried about the level of epi versus the atropine. Yes, you do have to worry about toxicity with atropine but the primary ...

Alan, I fully disagree with you trying to push that increased FIO2 should not be used in pediatric arrests. For one, Retinopathy of Prematurity and BPD are both medium and long term consequences of exposure to high flow oxygen over several hours a...

I never saw THAT in the ACLS protocols! All I can say, is that a Dr. works under his own license. All I could find regarding Atropine toxicity in peds is that as much as .5mg can begin toxic effects due to the heavy bolus. This child was given 1mg...

When working a peds arrest, if you're reaching for the epi or the atropine, you're generally doing something wrong. Without going into specifics it sounds like a cookbook medic panicking and doing all they know how to do.

I think I would want to know what the cause of the initial cardiac arrest was to put the 3 yo in that position. If the child was already in arrest, and I assuming (just based on epi and atropine given) that he/she was asystole. Atropine isn't reco...

maybe i am looking for demons, we are all going to make mistakes, this mistake was made over and over. OH, get this I have the ER report, the doctor noted "this three year old did not respond to ACLS protocols" I am going to keep searching for som...

Since we weren't there, and since treatment was directed by someone with more training than either of us, I think we should consider the possibility that there's something we don't know about the case. Lane Doby said:The treatment was wrong, rega...

The treatment was wrong, regardless of who directed it. Mike Rubin said:I'm not defending the dosing that you're reporting, but I don't think you can conclude that they were fatal. High-dose epi was once thought to be therapeutic in pediatric arr...

Not being a doctor, and not being involved in the case, I'm reluctant to second-guess. It's possible that an error was made. I've witnessed cases where non-standard treatment was based on the attending physician's experience with the presenting p...

what special considerations can you think of, even for organophosphate poisoning that is more than recomended. I am reluctant to talk to the doctor, we might get in an argument. I dont think they(code team) relized what they had done until it was ...

I understand your concern. Are you able to ask the doctor about the dosing? Perhaps there were special considerations for that patient. jim holt said:Yes a doctor was involved, I just can't believe that much medication wouldn't have contributed t...

Yes a doctor was involved, I just can't believe that much medication wouldn't have contributed to morbidity. Mike Rubin said:I'm not defending the dosing that you're reporting, but I don't think you can conclude that they were fatal. High-dose ep...

It wasn't me, I wasn't present, I need to know what effect that much atropine would have on the body. Can you can point me in the direction that will tell me. Alan Fijalkowski said:WHAT!? I would LOVE to know where this happened, and what is being...

As a newly-licenced paramedic, I also agree that the current system is flawed. My clinicals alternated between extremely good and a few situations where I was the most-senior person on the truck...as a student. The problem isn't limited to the civ...

doc, You are also forgetting the relative call volumes and the effect they have. Law enforcement is the busiest function (crime fighting) so the cops have the least time to devote to other, non-crime-fighting functions. That makes it intutively o...
