So, the company I work for happens to be an authorized training center for the UMBC CCEMPT program. Every year or so they open applications up for paramedics who want a spot in the next CCEMPT class,…Continue

John Gaines replied to Bob Sullivan's discussion Is oxygen hurting the patients we intubate?
"Most of the people harmed by field ETs are not harmed by O2. It's not oxygenation that is the problem. The problem is that "we" routinely FUBAR ventilation. Somebody gets all happy excited and starts hyperventilating grandma @88…"

John Gaines replied to ProfessionalEMS's discussion Is pain management really a big difference when comparing Paramedic to EMT?
"I agree with Dr Ex. It's rare when I am truly able to save a life. But I have the opportunity to ease someone's pain on an almost daily basis. I'm not giving it away like I was a predatory with candy or anything, but I have no fear or…"

John Gaines replied to Glenn Pratt's discussion Safer Ambulances needed!
"Hit the nail on the head there B.J. We can make ambulances as safe as we want (and yes, that's a good thing), but unless we are properly training drivers (and stop running crews for 24+ hours non-stop); then it doesn't matter one damn bit."

John Gaines replied to Earl Culvey's discussion Medication Shortages
"To the best of my knowledge the Epi and D50 were not drug shortages so much as a packaging shortage. One of the major companies that packed the pre-filled syringes stopped doing it (maybe went out of business) without warning. And let me tell ya,…"

John Gaines replied to NAI's discussion Video or Photos on MVC scenes?
"I'm all for it's use in certain situations. MVC's to be one of the big ones. For years I've used my cell phone (because there was on other option) to take a picture of the vehicle/scene to show to the doc and trauma team at the…"

John Gaines replied to Taegan's discussion EMS Personel Carrying Weapons
"It's about understanding the level of the threat in order to have a proper response. And that is why I strongly advocate self defense training. Look at it like the airway and our devices. We have cannulas, NRBs, npa, opa, bvm, combi, king, ET…"

John Gaines replied to Joe Paczkowski's discussion AHA Emergency Cardiac Care 2010
"If you could please post a picture of your WTF Face, that would be awesome. Perhaps it can be used as page 1 of the new Brady paramedic text book. If nothing else, it could make inside ambulances world wide. "

John Gaines replied to Taegan's discussion EMS Personel Carrying Weapons
"I never said DT4EMS was complicated. I said most EMS geared self defense programs I had seen teach techniques that are overly complicated. And as well stated I would research dt4ems. After I do, then I'll draw my conclusions."

John Gaines replied to Jennifer Berry's discussion Do You Tweet at Work?

dr-exmedic replied to John Gaines's discussion Violence List - Is Your Area On Here?
"Notice that everywhere is ranked by violent crime rate. If a place with 2 crimes has 100 people, it's going to get a higher rate than 19 crimes in a neighborhood with 1000 people. For proof of the silliness, note that there's not a single…"

Sometimes it's real

Stay in this business long enough, and it'll happen to you. Stay in even longer, and it happens a lot. Stay in even longer, and you'll have used the phrase "are you (explicative deleted) kidding me? Just stay in until you see the judge".

That's right boys and girls, I'm talking about "incarceritis". For those unfamiliar with incarceritis, it is a condition caused by being tossed into the hoosegow. Signs and symptoms usually include chest pain, "walking seizures" and/or having "the…
