Hello Sir. I see you are an EMT and a Medical Student. Would you have any idea about chest pain? I have been having painful squeezing in me chest and thorax that lasts up to 25 minutes. My arms don't hurt, just my body feels tight and its difficult to breath in. I have it at random times and I am just trying to see if anyone has any idea what it could be? Thank you sir

Joe, I like your concern, and drive, to improve EMS and your medical background will help. Jim and I had a similar conversation discussing your concerns about JEMS and change in EMS. Jim had ideas but he approached it differently than I did at the time. Not as much what should we do but who can do what? I can share more of that discussion with you off line if you like. Let me know and I will give you my email address. But I have question for you, which will change EMS more: cutting edge science or the basic principles? What if you decided the education requirments for EMS?

If you were suddenly put in charge of EMS tomorrow, would you change the education requirements? How? Would you standardize education across the country or would you continue to allow the piece meal certification style (e.g. an "EMT-Paramedic" in Iowa has 300 hours post EMT-B training whereas Oregon requires an associates degree. What's in a title?

A recent (re)posting of a sappy "I'M AN AMBULANCE DRIVER" 'rant' thingamajig on one of the several forums I frequent (all too often) got me thinking, "Why do people care so much about what we're called?" This isn't to say that what we proclaim ourselves to be is unimportant, but the amount of personal offense some people take when being called by some titles is astonishing.
