Hello all! Long time EMT-B, in class for EMT-I. Old age has made me give up the FF but still qualed as wildland FF. Here in Maine we
are mostly rural with some transport times over an hour on the ground.
In my day job I have access to helos (haven't had to transport anyone
yet) but am always looking for how folks handle being a long way out from definitive care.

Aaron, would love to hear more on your first on scene MCI!

I just became EMT-B, 15 year FF, the wicked physical FF stuff is good for crazy young ones! The Mohawk Valley in eastern NY is where I call home. Small villages and dairy farms, and a lot of seasoned citizens are what's here. My interest in EMS began after relocating a few miles and joining a new VF&RD. We have a great group (young & old) and several well experienced EMT's. The outdoors is my second home, doesn't matter what I do as long as it's outside. Growing my EMS skills has been truly rewarding and a great confidence builder, I look forward to growing those skills for as many years as the big guy has planned for me.

I am a full-time EMT-I with Kiowa County EMS in Greensburg, KS. This is the town that got taken out by the EF-5 tornado in May 2 yrs ago. I was here that night, and as much as i wished it wouldn't have happened, it was a great learning experience, me and my partner that night set up the first triage unit until more people arrived and took that over. It was cool to actually get to use some MCI training. Kiowa County is a big county, sometime its 35-40 minutes to a call way down in the badlands of the county. We do a lot of transfers out of here, we work out of trailers and tents, for our hospital right now until the new one is finished being built (December 2009 hopefully). I love my job and hope to finish paramedic school within the next couple of yrs. We also work very close the the local flight services, doin many scene flights. My town is on HWY 54 which is known as bloody 54, very busy highway. Anyway, out town is coming back strong and EMS is right on board with that. I love meeting new people and places so give me a shout!!!

I've a Volunteer EMT-B and (use to be firefighter). We cover 95 square miles of EMS service. Our service is ALS in Eastern Panhandle of WV. Our longest run can be 1 hour to a medical facality in VA. or 45 minutes to a medical facality in MD depending on which hospital the patient wants to be transported to.. If we get a call in the remote area of Levels, it is 1 hour to a VA. hospital or 45 minutes to a MD hospital. Our closest hospital is 10 minutes away, but not equipped to hand any major trauma, pregancy, etc. We mostly transport to the MD hosptial, there are two hospitals that we can go to there. If it's real bad trauma, we fly our patients to the MD facality. So we have lots to time to work on patients. I've been doing this type of work for 30 yrs and still love doing it. I enjoy helping people.

Our service is ALS in the Upper Peninsula of Michigan. We run two paramedic/EMT units to cover our county and intercept with surrounding BLS services. Coverage can get sticky at times, with long distance transfers and a small service group, but that seems to be norm with rural EMS.

I am an EMT-volinteer Firefighter. we are so rural that our town has a pop. of 298 people. But our area is MUCH bigger than that ! We work alot of elderly, hunting and atv accidents and a number of ods. not to mention FIRES. we have our shair of those too. but I love it or I would just stay home. We've had some really ugle car wrecks on some of these back roads. We have the fastest responce time in Franklin Co. It may be due to the fact that 98% of our members live less than a mile from the fire dept.

Since it's obvious to all of us that patient-centered, data driven, evidence based, and clinically excellent EMS is worth striving for, regardless of who provides it, maybe we could work together and come up with a thread entitled "characteristics...
