I need some help. I am attempting to get a community wellness program off and running at my firehouse. I want this program to entail home visits with our frequent flyer patients and our under privileged population. Initially i wanted it to included basic vital sign monitoring including glucose monitoring for our diabetic population, insuring compliance with medication and treatment and answering any question the patient would have in regards to treatment or their disease process. I was wondering if anyone would have any advise on how to best get this program off the ground or any suggestions on other things we should look at incorporating. Thank in advance for the input.
